Background: Patients with concomitant adrenocortical carcinoma and myelolipoma are extremely rare and to the best of our knowledge there has been only one case reported in the available literature.\nWe present a case of incidentally discovered nonfunctioning adrenocortical carcinoma with concomitant myelolipoma. The patient was a 72-year-old man with benign prostatic hyperplasia, severe prostatitis and cystitis, for which he paid a visit to a physician. \nMaterial and Methods: Echosonography revealed left adrenal gland mass confirmed by computed tomography. The patient underwent unilateral adrenalectomy and operative material was sent to the Institute of Pathology in Skopje for histological analysis.\nResults: The adrenal gland weighed 113 g and measured 8 x 6 x 3.5 cm. On cut surface a multinodular partly well and partly poorly circumscribed tumor mass measuring 5.5 x 3.5 x 3.5cm was seen. It was partly light brown and partly white-gray colored. A central hemorrhage was present in more than one half of the tumor which revealed microscopically to be adrenocortical carcinoma. On microscopic examination we found cortical carcinoma showing cytologic atypia, necrosis, hemorrhage, 6 mitoses/10 HPF and venous invasion in two thirds of the tumor mass and collision myelolipoma in one third of the tumor. \nConclusion: Although extremely rare an adrenocortical carcinoma with concomitant myelolipoma should be included in the differential diagnosis of adrenal gland masses.

Satellite communication systems e.g. digital video broadcast satellite/terrestrial (DVB-S2/T2) have readily adopted LDPC code for Forward Error Correction (FEC), mostly due to its near Shannon performance at very low signal to noise. This paper summarizes the differences across FEC subsystems adopted in worldwide standards for next generation fixed satellite broadcasting. The goal of this analysis is to provide system designers an objective view of the different choices made in these standards and facilitate an easier system implementation for the same.

To logically prescribe the optimized hydrolytic parameters (here beta-glucosidase (BG), cellobiose dehydrogenase (CDH), endoglucanase (EG), xylanase (Xyl), and metabolic coregulaters) of nature-friendly specialized biodepolymerization (without any attached redox systems) by Dichomitus squalens, the long-term patterns of enzyme biokinetics were interconnectively evaluated based on the multivariate forms of modified unstructured models. In especial, based on the metabolic fluctuation by open structures (via peroxidative cascades), the proposed disruptive models were roughly well-managed as the mild correlations (below 59%) between theoretical predictions and actual mainstreams. However, remarkably, the biomass utilization kinetics (either cellulose or hemicellulose; over 97.4%) by the collaborated cascades may be significantly able to increase the universal validity that the expanded functions is useful for the long-term fungal deconstruction. The updated information can be pragmatically used to systematically carry out the fundamental realignment of consolidated bioprocessing systems for lignocellulosic biofuels.

The goal of this paper is to provide an extensive review of the approaches and techniques, developed to solve multiple objective job shop scheduling problems during the last half century. These scheduling problems are a challenge for researchers because they find a wide application in the practice. Further, they are difficult from computational point of view. The development of multi-objective methods for solving job shop problems is relatively new area. Most proposed methods consider two or three objectives functions. The proposed algorithms are close connected with the selected objectives. In this survey the methods are classified as exact methods and heuristic methods. Some specific approaches and hybrid methods are discussed also. Many metaheuristics, as well as nature inspired evolutionary algorithms, developed to solve real life job shop scheduling problems are summarized and their advantages and disadvantages are considered in the paper.

Many villages in Egypt suffer from energy shortage and proper disposal of municipal solid waste, which presents chronic sanitation problems. The main objective of this research is to prodcuce sufficient amount of biogas from the kitchen organic waste, agricultural waste and black sewage water to be used in cooking and heating at village homes, through designing a 1000-liter bioreactor using digestion enzymes that help in decomposition of complex organic wastes in anaerobic media to produce biogas contains approximately 55 to 65% methane gas content in less than 21 days with high efficiency. \n\nThe calorific value of biogas is appreciably high (around 4700 kcal or 20 MJ at around 55% methane content), the designed device can be safely used at homes as it is odorless and the effluent supernatant digested waste water produced from the digester can be connected directly to the sewer system after being digested or can be used after aeration on land application as fertilizer.

Objective of this study: the aim of the current study is to assess the readiness of the physicians in Al-Hada Military Hospital in Taif city towards implementing EMR.\nMaterial and methods: Questionnaire consisted from Personal Demographic variables, Practical Demographic variables, Beliefs and Attitudes variables, and Security and Confidentiality variables were distributed on 131 Practicing Physician, Accepting to participate, Present at the time of the study (not on vacation or leave),and Working in Al-Hada Military Hospital in Taif city (excluding visiting or locum physician).\nResults: Out of 129 physicians, 107 were males which represent 83% while females represented only 17%. Most of the participants were from Family and Community Medicine (25%), followed by Surgery (20%), Pediatrics (14%), Medicine (12%) then OBG (8%). Other departments represent almost 21% cumulatively. Most of physicians had an encouraging awareness and beliefs about the potential benefits of implementing EMRs. About two third of physicians believe in easiness of using EMRs and only 4% had expressed their need for intensive training.\nConclusions: Generally, the attitudes of Al-Hada Military Hospital physicians towards computerization of medical record are excellent. These positive attitudes fall in favor of adopting EMRs.

The main etiological factors causing apical periodontal lesions are the bacterial populations located in the main canal and in dentinal tubules, lateral ramifications and apical resorption areas. The success of endodontic treatment is highly dependent on thorough eradication of bacterial species or at least their reduction to levels predisposing healing of periradicular tissue. \nChemo-mechanical preparation of root canals is not sufficient to eliminate all bacterial species and their toxins, that’s why antimicrobial dressings have been advocated. Calcium hydroxide is a medication of choice and is preferred for its antibacterial, mineralizing and anti-inflammatory properties.\nThe results of endodontic treatment of four cases with asymptomatic chronic apical periodontitis were traced for three years. Patients’ condition was evaluated with clinical examinations and regular X-rays. The treatment protocol was one and the same for all patients. The good long-term results achieved in this study are due to the combination of extensive mechanical preparation, irrigation with sodium hypochlorite and inter-appointment dressing with calcium hydroxide. \nIn conclusion, we find more rational to complete treatment of chronic periapical lesions in two sessions, applying a calcium hydroxide dressing for 14 days. Long-term monitoring is mandatory as treatment outcome is not always satisfactory and stable in time.

When a SOAP Web Service is receiving an encrypted\nXML document, it decrypts the key and uses it to decrypt all\nthe parts of the document containing encrypted data. Finally, the\ndata are parsed with an XML parser and the whole document\nis forwarded to the next module. In this case, the XML Security\nprocessing module typically does not know which parts of the\ndecrypted data are later processed by the business logic due to\nthe absence of time-stamp during the decryption process. Thus,\nit could also happen that the encrypted data are decrypted,\nparsed, and not processed further for security analysis. In this\nresearch, we use a temporal model for XML representation\nusing a combination of valid time and transaction time through\ntuple time-stamping. In addition to the time-stamp, we use\none attribute of the key components of the encrypted elements\nto distinguish among them. The time-stamp and the selected\nattribute will guide the receiver to decrypt the required parts\nof the encrypted XML documents instead of decrypting all the\nparts. Moreover, they can be used to retrieve the parts that\nmust be decrypted to contribute to the query results over the\nencrypted XML documents. The proposed technique not only\nreduces decryption time, but also it increases the security of the\ndata.

This paper proposes an Improved Collision Avoidance Strategy (ICAS) based on an Autonomous Vehicle Routing Protocol (AVRP) for better safety of a human life. The proposed ICAS is designed in the following order. First, it measures the density of vehicles and predicts their location, by collecting their location and speed information. Second, it establishes a robust message transfer route with the predicted location of vehicles. Third, it transfers a Collision Alert Message or a Collision Prediction Message through the route in case the distance between vehicles is within a safety distance. The experiment shows that the PDR (Packet Delivery Ratio) of the proposed AVRP is enhanced by 42%, 28%, and 2%, compared to the existing AODV, DSDR and GPSR, the E2ED (End to End Delay) of the AVRP is reduced by 35% and 6%, compared to AODV and GPSR, and the Average Throughput of the AVRP is improved by 29%, 45% and 2%, compared to the AODV, DSDR and GPSR. Therefore, the ICAS enhances a driver’s safety highly by accurately and rapidly transferring a Collision Alert Message.

5-Amino-3-methyl-1-phenyl-1H-pyrazole-4-carbonitrile (1) was used as a precursor for preparation of 3-methyl-1-phenyl-1,5-dihydro-pyrazolo[3,4-d]pyrimidin-4-one (2) and its derivatives 3-10. The produced compounds were separated, Purified, and characterized by FT-IR, 1HNMR. The surface properties were studied by measuring the surface tension. The surface tension, critical micelle concentration (CMC), and surface activities were determined. The surface parameters such as surface excess concentration (?max), the area per molecule at interface (Amin) and the effectiveness of surface tension reduction (? CMC) were determined from the adsorption isotherms of the prepared compounds. Furthermore, the corrosion inhibitors of the prepared compounds was evaluated by chemical methods(weight loss) at different inhibitor concentrations and temperatures. The corrosion inhibition efficiency increased with increase in inhibitor concentration, but decreased with increase in temperature. Introduction: There is lacking of evidence regarding psychological distress that occur among expectant fathers during ante-partum period in Malaysia. The objective of this study was to determine the prevalence of psychological distress among Malaysian expectant fathers in one Goveronmental Medical Center.\nMethods: This was a cross-sectional study involving 124 couples. They were recruited via systematic random sampling at antenatal clinic within 2 months period. The Depression Anxiety Stress Scale 21 (DASS 21) questionnaires in both English and Malay versions were administered to screen for any psychological distress namely depression, anxiety and stress. Data collection form was used to obtain socio-characteristics of the respondents.\nResults: Median age of fathers in this study was 32 (7) years. Almost four fifth were Malay (78.2%), followed by Chinese (14.5%) and Indian (5.6%). More than one third (36.3%) out of 124 expectant fathers were detected to have psychological distress. The prevalence of stress, anxiety and depression were 13.7%, 33.1% and 12.1% respectively. Psychological distress among expectant fathers was significantly associated with maternal depression (p=0.001). Moreover, being first time father was related to depression state (p=0.039). \nConclusion: There is high prevalence of psychological distress among expectant Malaysian fathers during ante-partum period. It was associated with their spousal’s depression. Inexperience fathers are linked with negative psychological state such as depression. Hence, healthcare providers should take the opportunity to explore emotional and psychological state of the expectant fathers while accompanying their partners for routine antenatal care.

To logically elucidate the essential ligninolytic parameters (here manganese peroxidase (MnP), laccase (Lac), glyoxal oxidase (GLO), aryl-alcohol oxidase (AAO), and hydrolytic intermediates) of eco-friendly biomass biofermentation (without downstream inhibitors) by Dichomitus squalens, the long-term expression formats of enzyme kinetics were mathematically analyzed based on the modified forms of multivariate unstructured functions. In particular, the advanced deconstructive model (approximately 98% associability based on total biomass) by combined ligninolytic controllers had the potential to stably maintain the kaleidoscopic networks of downstream kinetics, and so this MnP-based biosystem was well-managed as the high correlation (R2 = 0.8700) between theoretical mainstream and actual observation. However, the induction patterns of oxidative cascading mechanisms (here Lac, GLO, and AAO; below 69%) were restricted in interrelation with each other. The updated kinetic results may be used to provide the monitoring information on fed-batch feeding as well as to verify to the methodological bridge of consolidated bioethanol program.

Recycling of the waste cooking oil (WCO) to produce nonionic surfactants via solvent free efficient green synthesis. In this respect, the waste cooking oil ( Triolein) was collected, Purified, then reacted with ethanol amine (EA) and ethylene glycol (EG) in the presence of sodium acetate, manganese acetate, as a catalyst, to give Octadec-9-enoic acid (2-hydroxy-ethyl)-amide, (ODHA) and Octadec-9-enoic acid 2-hydroxy-ethyl ester (ODHE) respectively as nonionic surfactant. Sulphur and nitrogen containing organic compounds are gaining importance in synthetic\nand pharmaceutical fields. Thiazoles are a heterocyclic organic compounds are well known intermediates in thesynthesis of clinically important heterocycles such as 4-thiozolidinones and benzothiozoles.\nThiazoles are found in medicaments like vitamin-B, sulphathiozoles, promizole, niridazole,\naminotrizole and tetramisole. Therefore, and because of the importance of these compounds, the \nunderstanding and study of the mechanism and kinetic of their reactions are particularly important.

Narrowleaf plantain (Plantago lanceolata L.), marigold (Calendula officinalis L.) and shepherd’s purse (Capsella bursa-pastoris L. Medik) are really popular, useful herbs for several diseases because of their considerably high and diverse agent- and element content. The manganese and iron content were investigated in their paramagnetic form (Mn(II) and Fe(III)), as these oxidation states are very important in human metabolism. The paramagnetic forms of these ions were detected by Electron Paramagnetic Resonance (EPR) spectroscopy. For the quantitative analysis of Mn(II) the EPR signals were simulated by a computer program and the calculated double integral were compared with those of standard calibration series. In this way an effective quantitative method was implemented in order to determine the absolute Mn(II) content, which can be evolved later as a rutin technique. The Mn(II) content of the infusums were measured and these amounts were compared to the total Mn content determined previously by ICP-OES. It gives rise to the possible use as dietary intake for Mn(II). The Fe(III) content of the samples were investigated by frozen solution EPR measurements comparing these spectra with different Fe(III)complex standard solutions. The results pointed that Fe(III) may exist in Fe(III)citrate complex in the solutions. The concentrated, frosted infusums were prepared to reach a better sensitivity. From this kind of sample Cu(II) could also be detected beside Mn(II) and Fe(III). Fe(III) content was also determined in ground drug samples. Marigold was carried out to be the richest plant in Fe(III), and the next one was narrowleaf plantain.

Linearly Constraint Minimum Variance (LCMV) is a beamforming method that directs a radiation beam to the desired direction and decreases the signal’s interference via weightvectors calculated by LCMV. Weights of LCMV are usually not precisely directed to the direction of user to achieve a desired beam shape. Moreover, it is not easy to improve the weights of LCMV using traditional methods. In this paper, weights of LCMV are optimized using Firefly Algorithm (FA) and Particle Swarm Optimization (PSO). The main desired goal in the optimization of LCMV is to minimize the power on the side of interferences that leads to better SINR output and a favorable beamshape. Particle Swarm Optimization (PSO) is a popular metaheuristic algorithm based on social behavior of creatures like bird flocking. Meanwhile, Firefly algorithm is one of the latest swarm intelligence metaheuristics inspired by the flashing characteristics of fireflies. The simulation results show that FA in compare with PSO, possesses better ability to accurately determine the optimal direction for power allocation and, hence, produces better SINR results. MATLAB® was used to simulate the effectiveness of FA in comparison with PSO algorithm for SINR optimization using LCMV technique.

The article is describing the start of experimental analysis of temperature differences under different types of sedum growing on a green roof. The test is carried out on a green roof involving one type of growth medium with three sedum types (Sedum Diffusum \'Potosinum\', Sedum Spurium \'Voodoo\' and Sedum Kamtschaticum). The temperatures are being recorded every 30 minutes. The variations of temperatures should show what kind of growth media and precisely what kind of vegetation can yield significant differences temperatures, thus energy savings and water retention.

A series of new 2-substituted pteridine derivatives have been prepared by reaction of 2-mercapto-6,7-diphenylpteridin-4(3H)-one with chloro-heterocyclic compounds under basic medium. Introduction: The main objective of this study was to understand the impact of caffeine upon adolescents’ health and health related behaviours, namely sleep duration and health related quality of life. \n\nMethods: In this study participated 3869 Portuguese adolescents, of which 46.8 % boys, attending grades 8 and 10 (61% in grade 8), mean age 14.7 years, SD =1.18, included in the Portuguese HBSC survey 2014. In a first step descriptive analyses were carried out and subsequently three regression models were evaluated and showed good statistical fit. \n\nResults: During weekdays the consumption of coffee and soft drinks with caffeine seems to impact the sleep duration. Both sleep duration and quality have an important impact upon health perceived quality of life. Drinking soft drinks with caffeine and computer use have poor impact. The use of energetic drinks with alcohol was relatively low; it had no impact upon sleep duration either during weekends or weekdays, but the impact upon the perception of health quality of life was negative. Gender and developmental/ age differences were found. \n\nConclusions: Caffeine intake has negative impact upon sleep duration and perception of health related quality of life. The implications for health promotion interventions in families and in schools are discussed together with implications for public policies in the area of sleep, perception of health related quality of life and caffeine intake.

Abstract\nIntroduction: An association of thyroid function and cognitive impairment has not been established . To avaluate the issue this large population based study is designed . \nMethodology: This study is part of a comprehensive plan for the Amirkola Health and Ageing Project (AHAP).The level of TSH, T3, T4 was examined using the fasting blood samples and mental status evaluated by MMSE (Mini Mental State Examination) standard questionnaire. Then the data were analyzed by SPSS18 software.\nResults: 1607 patients with a mean age of 68.2±6.97 were included in the study, among which506 (31.5%) had symptoms of cognitive impairment. There was a significant relationship between T3, TSH and MMSE. \nConclusion: There was a significant relationship between T3 , TSH and symptoms of cognitive impairment . but it was not significant in logistic regression model. Due to these controversies mush more studies is sujjested .

Studies on the breeding biology of catfish, Rita rita (Hamilton) was carried out for the period of 12 months from June 2014 to May 2015. Fish samples were collected from River Indus, Jamshoro. The size of samples were ranged from 36.0- 41.0 cm and weight about 450 – 800g. The values of the parameters such as Ova diameter, Fecundity and GSI were recorded. It was indicated from the finding that the indigenous catfish, Rita rita (Hamilton) breeds once in year during the months of May to July with peak in July as mentioned by both values of GSI and Ova diameter. When \"t\" test was applied it found highly significant (P< 0.01) level the fecundity bears a strong relationship with gonad weight in this fish. It is concluded from the above findings that the full grown specimens about 1 kg weight could produce about 0.2 million eggs during the breeding season.

This paper proposes a new modulation technique that doubles the transmission rate of data symbols in digitally modulated systems. The rate is doubled without any changes in the used signal constellation or the modulated signal bandwidth. The proposal doubles the transmission rate by using Complex-Exponential-Basic-Functions (CEBFs) instead of Real-Sinusoidal-Basic-Functions (RSBFs) in synthesizing of digitally modulated signal. If CEBFs are used as carriers, the time-domain samples of the modulated signals will be complex. Two orthogonal pulses reshape the real and imaginary parts of the complex-modulated symbols. The reshaped symbols are added to form one real-modulated signal. The reshaping pulses are chosen to be Hilbert-Transform pair. The paper shows that the canonical form of the modulated symbol in the proposed system looks like the canonical form of the Single-Side-Band modulation. The bandwidth efficiency of the proposed scheme is double the bandwidth efficiency in systems use RSBFs.

Organizations continually try to increase the level of leadership capability and increase the number of qualified applicants for leadership positions by supporting leadership development training program. This qualitative case study discovers whether participating in a leadership development program resulted in leader’s performance that can fill leadership gaps. The theoretical framework of this study is depend on expectancy theory, suggests that leaders who involved in leadership development expect to become leaders in future. This exploratory case study sought to learn whether, upon completion of a comprehensive leadership development program, participants applied for, and assumed, leadership positions. A purposeful sample of 32 participants was drawn from in structured interviews conducted by the researcher from Aramco Company. The results provided evidence that using leadership development programs to fill a leadership gap is productive, and that the effectiveness of this strategy is enhanced when organizations purposefully select and support participants through all stages of their leadership development. Participants who pursued leadership opportunities indicated the importance of institutional involvement in leadership development.
